RETURN MERCHANDISE AUTHORIZATION FORM

Requests for return of any item must be expressly authorized by Alta Power Corp, within 30 days of the original delivery receipt date. Any item returned to Alta Power Corp without an approved Alta Power Corp RMA # will be refused by our warehouse and automatically returned to the recipient.
